Job Summary:

The LPN within inpatient and ambulatory dialysis units cooperates to deliver direct and indirect patient care, acting under the direct supervision of a physician or RN. They administer nursing care to allocated patients and assume responsibilities delegated to address patient needs. The LPN provides nursing care in accordance with Monument Health's guidelines and processes, professional standards, the South Dakota Nurse Practice Act, and other applicable federal, state, and local regulatory or accrediting bodies.

Job Description

Essential Functions:

  • Illustrates the aptitude to effectively complete and record a focused assessment, apply clinical interventions, evaluate the outcomes, and adapt the care plan in collaboration with a registered nurse or healthcare provider.
  • Readies themselves for patient care by engaging in pre-visit planning and necessary preparations.
  • Accurately manages and records medication administration according to departmental policies and guidelines.
  • Executes nursing treatments and practices within the Scope of Responsibilities as per the standards established by the nursing department and the unique requirements of the setting.
  • Illustrates the competency and sound judgment essential for administering physician directives, protocols, and nursing procedures appropriately.
  • Shows the ability to perform and record assessments, utilize clinical interventions, evaluate progress, and make changes to the care plan while working closely with RNs/providers.
  • Supports the registered nurse in gathering data relevant to assessments, through activities such as reviewing charts, observing, or conducting audits, while applying suitable age-specific criteria.
  • Assists registered nurses throughout the nursing process by contributing to the nursing assessment, participating in care planning, helping to develop nursing interventions, and contributing to evaluating the responses to nursing interventions.
  • Issue preapproved patient education resources that have been preprinted. Offer additional reinforcement for health teaching and discharge instructions delivered by the nurse or provider, and redirect all patient inquiries to the nurse or provider.
  • Supports and enhances departmental development by actively participating in meetings, committees, and continuous learning opportunities.
  • Takes part in the onboarding and mentoring of clinical staff based on given instructions.
  • Validates the clarity and accuracy of information recorded in patient documents in line with the Scope of Responsibilities.
  • Other tasks as deemed necessary.

Additional Requirements

Required:

Education - Completion of a nursing education program that is approved by a board of nursing

Certification - Licensed Practical Nurse (LPN) - South Dakota Board of Nursing

Preferred:

Experience - 1+ years of Healthcare Experience

Physical Requirements:

Very Heavy work - exerting over 25 pounds of force constantly (67-100% of the time), and/or more than 50 pounds frequently (34-66% of the time), and/or more than 100 pounds of force occassionally (up to 33% of the time), and/or more than 100 pounds of force seldomly to move objects. Possible exposure to radiation and frequent exposure to contaminated needles and infectious body substances. Requires the ability to work designated shift lengths (including 8 and 12 hours), which may include night shifts or occasional irregular hours.
